Requirements

LOCALLY EMPLOYED

  1. Cert. of Employment and Compensation and 1 Month Latest Payslip
  2. 2 Valid IDs (photocopy with 3 specimen signatures)
  3. TIN ID or kahit yong number lang
  4. 5 pcs 1×1 ID pictures with white background
  5. NSO Birth Certificate (if single) or Marriage Contract (if married)
  6. 2 Valid IDs for spouse
  7. Proof of Billing ( any from light, water,  or cable bill)
  8. Approved MSVS from Pag-ibig ( kami na po ang mag-assist nito)
  9. Post Dated Checks (Endorsement letter will be provided)

Additional Requirements for Married:

  1. 1 Month Latest Payslip of spouse if working or any proof of income

SELF EMPLOYED

  1. Business Permit and DTI
  2. Financial Statement
  3. 2 Valid IDs
  4. 5 pcs 1×1 ID pictures with white background
  5. TIN ID or kahit number lang.
  6. NSO Birth Certificate (if single) or Marriage Contract (if married)
  7. 2 valid IDs of spouse if married
  8. Proof of Billing ( any from light, water,  or cable bill)
  9. Approved MSVS from Pag-ibig ( kami na po ang mag-assist nito)
  10. Post Dated Checks

WORKING ABROAD

  1. Latest Job Contract or 1 Month Latest Paylip
  2. 2 Valid IDs – Philippine issued IDs (Passport is one of them)
  3. TIN card or kahit yong number lang (If none, we can help you get one for you)
  4. 5 pcs 1×1 ID pictures with white background
  5. NSO Birth Certificate (if single) or Marriage Contract (if married)
  6. 2 Valid ID of spouse if married
  7. Special Power of Attorney (we will send you a copy via email)
  8. Approved MSVS from Pag-ibig ( kami na po ang mag-assist nito)
  9. Post Dated Checks (Endorsement Letter will be provided to your representative)
